Maintenance Program Notice:To ensure service eligibility, a minimum of four monthly payments must be completed prior to your first scheduled visit. For immediate service, a one-time payment covering these four months may be made at enrollment; otherwise, service will be scheduled once the fourth consecutive monthly payment is received. Maintenance visits are performed according to the visit frequency selected for each system in your agreement.
